7th Grader needs questionaire help

Hi,

I'm the father of a 7th grader who is interested in doing a science project on how color affects mood. The approach explained in this site is very helpful, but we're not sure how to compose the questionaire. Is there is a sample that you can send to me? We're not quite sure, but the survey group may be elementary school children, 6th - 8th graders.

We would appreciate your help!
