This sounds like it is going to be an interesting project.

Some items that need to be addressed:

Designing a survey that meets all of your goals is difficult. I'd like to see this survey as soon as possible.
Just what are some of the assumptions our society has about beauty? Hit the literature and find out! Make up your survey after you have obtained your sources. Believe it or not, these sources can help you design a better survey instrument.
Are their differences between society's image of what is beautiful and one's own self image?
How does the concept of beauty compare among other cultures?
